Terms & Conditions

Effective date: August 28, 2026

These Terms & Conditions (“Terms”) govern your access to and use ofconciergemm.com (the “Site”), which is operated by Concierge Medical Management (“CMM,” “we,” “us,” or “our”). By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

1. Website Purpose

The Site provides general information about CMM’s concierge medical practice startup, operations, administrative support, technology, payment, and marketing consulting services. Site content is provided for general informational and business-development purposes only.

2. No Medical Advice or Patient Relationship

CMM provides business and operational consulting services to medical practices. The Site does not provide medical advice, diagnosis, treatment, or emergency services, and use of the Site does not create a physician-patient or other clinical relationship. Always seek advice from a qualified healthcare professional regarding a medical condition.

If you are experiencing a medical emergency, call 911 or your local emergency service. Do not use this Site or its contact form for urgent or emergency communications.

3. Do Not Submit Patient Information

The Site’s general contact form is intended for business inquiries. Do not submit medical records, patient names, dates of birth, diagnosis or treatment information, insurance information, or other protected health information (“PHI”) or sensitive personal information through the Site.

Submitting information through the Site does not create a business associate relationship or business associate agreement. Any exchange of PHI in connection with an established consulting engagement must use the secure process and written agreements separately approved by CMM and the client.

4. Consulting Engagements

Information on the Site is not an offer or guarantee of services, pricing, outcomes, or availability. Sending an inquiry, scheduling a call, or communicating with CMM through the Site does not create a consulting, fiduciary, professional, or contractual relationship. A consulting engagement begins only after CMM and the client sign a separate written agreement. If that agreement conflicts with these Terms, the signed agreement controls for the consulting services.

5. Your Use of the Site

You agree not to:

6. Intellectual Property

The Site and its original text, graphics, logos, photographs, design, and other content are owned by or licensed to CMM and are protected by applicable intellectual property laws. You may view and print reasonable portions of the Site for your own internal, noncommercial business evaluation. No other copying, modification, distribution, republication, sale, or exploitation is permitted without CMM’s prior written consent.

7. Third-Party Services and Links

The Site may reference or link to third-party websites, platforms, products, or services. CMM does not control and is not responsible for third-party content, availability, security, accuracy, or privacy practices. A reference or link does not necessarily imply endorsement. Your use of a third-party service is governed by that provider’s terms and policies.

8. Privacy

Our Privacy Policy explains how information submitted through or collected by the Site is handled. The Privacy Policy is incorporated into these Terms by reference.

9. Disclaimers

To the fullest extent permitted by law, the Site is provided “as is” and “as available,” without warranties of any kind, express or implied. CMM does not warrant that the Site will always be available, secure, error-free, or complete, or that Site content will meet every user’s particular business, legal, regulatory, financial, technical, or clinical needs.

Healthcare, privacy, employment, payment, and professional-practice requirements vary by jurisdiction and circumstance. You are responsible for obtaining advice from your own legal, compliance, tax, accounting, clinical, and other qualified professionals before acting on Site information.

10. Limitation of Liability

To the fullest extent permitted by law, CMM and its owners, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, revenue, data, goodwill, or business opportunity, arising from or related to your use of or inability to use the Site. Nothing in these Terms excludes liability that cannot lawfully be excluded or limited.

11. Changes to the Site or These Terms

We may update the Site and these Terms from time to time. Updated Terms will be posted on this page with a revised effective date. Your continued use of the Site after an update means you accept the revised Terms.

12. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any dispute relating to the Site or these Terms will be brought in a court of competent jurisdiction in Florida, unless applicable law requires otherwise.

13. Severability and Entire Agreement

If any provision of these Terms is held unenforceable, the remaining provisions will continue in effect. These Terms and the Privacy Policy constitute the entire agreement between you and CMM regarding use of the Site, except for any separate written agreement governing consulting services.
